DIRECT CARE PROFESSIONAL/CAREGIVER

Job Summary

LOCATION: Harvest, Alabama

The Direct Care Professional provides services for residents by following a specific plan of care identifying assigned goals and tasks.  A DCP participates in activities to safeguard the health and wellness of the residents.  Direct Care Staff provides personal care to persons with intellectual disabilities in a residential setting.  Duties include but not limited to daily living activities, personal care/hygiene.  This position is responsible for the care, training, activities  and supervision as needed of persons with intellectual disabilities. Must have the ability to understand instructions, maintain records and write reports.

ESSENTIAL DUTIES: (not limited to)

Prepare and serve meals and snacks; include residents in community outings; accompany residents on shopping trips; observe and document behavior; may give medications as prescribed and trained.

REGISTERED NURSE

Job Summary

LOCATION: Harvest, Alabama

The Nurse provides medical services for residents that will increase the quality of life for persons served. The Nurse will lead and train our staff in providing quality care as it relates to the medical needs of residents.

ESSENTIAL DUTIES: (not limited to)

Obtain and maintain MAS certification as delegating nurse from the Alabama Department of Mental Health’s Nurse Delegation Program (NDP).

Available for ON-CALL assignments.

Provides skilled nursing care.

Provides training and supervision to MAC workers providing direct medical care to residents.

Administers prescribed medications.

Completes medical treatment as ordered.

Oversees and complete accurate and detailed documentation of resident records.

Facilitates and monitors staff performance and competency and recertification skills.

Supervises and evaluates ongoing performance of Direct Care Professionals (DCP).

QUALIFIED INTELLECTUAL DISABILITIES PROFESSIONAL (QIDP)

Job Summary

LOCATION: Harvest, Alabama

The QIDP position serves as the liaison among all disciplines. The QIDP is the center of all discussions with family, individuals, staff, guardians, administration, and case managers.

ESSENTIAL DUTIES: (not limited to)

Ensures Direct Care Professionals (DCP) are completing proper documentation and activities of individuals served.

Lead in developing and writing behavior plans and a variety of reports for individuals to meet state requirements.

Develop plan for individual goals.

Ensure all plans are being carried out.

Report all suspected and observed incidents of abuse or neglect of any individual served.

Schedule, lead, and write agenda for DCP trainings.

Maintain communication between individuals, families, guardians, doctors, therapists, residential supervisor, direct care, executive director and all other parties.

Attend trainings necessary to support individuals and staff.

Adhere to all other policies and procedures set forth by 1st Lady Empowerment Foundation, Inc. and WeCare.

Bachelor’s Degree preferred in Social Work, Psychology, or a related Human Services

Experience working with Individuals with Intellectual Disabilities a plus!
